Comprehensive Care Management

 

Is your loved one safe in their home?  Are there things that can be done to improve their safety that will allow them to stay at home? What options are there and how do you ensure your loved one is in a safe, quality environment?   At Q4, we are experts in evaluating the home environment, your loved one’s needs and abilities, finding housing solutions and evaluating them to ensure their services match your loved one’s needs. 


  • Disease Management

Being diagnosed with a disease is a challenging experience.  There are new physicians, life-style changes, and new medications.  Often times there are an array of emotions including sadness, anger, denial, and despair.  We feel victimized and out of control.  The disease does not have to control your life and you have the power to control your disease.  Your Q4 Care Manager can help you understand this experience and help you comprehend that you have a disease; the disease doesn’t have you. 


  • Symptom Control

It’s time for you to control your symptoms and to not allow your symptoms to control you any longer.  So often, we think that we have to live with pain, discomfort, or limited abilities.  This is so often not the case.  Your Q4 Care Manager partners with you and your physicians to find solutions.  We want you to live your best life and to live symptom free will help you achieve this goal.” OR “We want you to live your best life.   • Improving Function

Often, the result of disease and disability is limiting function. We may be limited in our ability to care for our home, our finances, or ourselves.  This threatens our independence and autonomy.  Many assume these changes must take place and nothing can be done to stop it or reverse the effects.  At Q4, our Care Managers are trained to see just the opposite; we look at what is limiting you and focus on improving your function.  Doing this has greatly enhanced our client’s quality of life; let us improve yours.


  • Medication Monitoring

Taking medications is often a mindless thing.  We have problems, we got to the doctor, and we are prescribed medications.   We take our medications faithfully, but often they don’t eliminate the problem and it is difficult for your doctor to determine whether drug interactions, time of day, or side effects are causing the problem.  Physicians are plagued with questions concerning lingering symptoms, new symptoms, or interfering medications.  It is complex process and your Q4 Care Manager will monitor for medication reactions, interactions, and side effects.  Your Care Manager will work with your physician and pharmacist to resolve the problems and restore your health.


  • Crisis Intervention

In a crisis, situations occur in rapid succession: there is an onslaught of new information, tests are performed, and procedures are completed.  There are new physicians and nurses needing you to make life-altering decisions.  It is difficult to decide who to trust and what decision is the right one for your loved one.  Dealing with the emotions of a crisis can be daunting.  Your Q4 Care Manager knows you and understands what is happening.  They are the rudder you need to navigate your ship in the darkness and bring you to a safe harbor.


  • Home Safety

Staying in the home we love is a common goal for most people.  Making it happen can be challenging and overwhelming as our health changes,  our ability to care for ourselves diminishes and our home becomes an obstacle instead of an oasis.  At Q4, we excel at investigating your needs, your home’s current condition and developing a plan to make the changes that allow you to stay safely in your home.  We look for the obvious and the subtle, all based on your abilities and limitations. 


  • Home Modification

Your home is not set up to accommodate your needs.  This threatens your independence and making major changes to your home can be daunting.  It doesn’t have to be this way.  Often times there are adaptive devices that can minimize the modifications to your home.  At Q4, we can help you evaluate your needs, your home’s ability to meet them and identify what adaptive changes can be made to give you maximum independence and maximum enjoyment.


  • Social Optimization

Little emphasis has been placed on socialization access and appropriateness. It is assumed that if you live near others, socialization will occur. Sadly, this is often untrue. You may have heard the saying, “I can be lonely in a room full of others.” Yet, research has shown that relationships are fundamental to health and happiness. Your Q4 Care Manager evaluates your desires, your interests, availability of activities, and then, facilitates your involvement.

  • Relocation Services

The decision has been made that your loved one needs to move, but developing a plan to ensure that process goes smoothly can be difficult and challenging.  There are different considerations depending on whether your loved one is moving locally or out of their city or state.  Q4 Care Managers are able to help you navigate these challenges, whether it is the physical move, the emotional considerations, or ensuring continued uninterrupted medical care. 


  • Travel Coordination

Traveling to visit friends and relatives is often an important goal for aging or disabled individuals. Planning and managing medical conditions while on vacation can be overwhelming and can put your loved one’s health at risk.  Arranging for transportation, housing, and accessible venues must be planned to prevent problems.  At Q4, we can take care of the details so you and your loved one can enjoy your trip.


  • Disaster Preparedness

Disasters happen when least expected and can have devastating effects.  Whether the disaster is an act of nature, like hurricanes, tornados, floods or fires, it is essential that there is a plan and course of action.  Housing concerns, food and water, warmth, and money are all important concerns but all too often planning for our health is overlooked and undervalued.  At Q4, we want you to be safe and we will help you create a plan.
