Things were changing and not for the good.  Her family noticed that she was more forgetful and easily exhausted.  She had fallen twice in the last couple of months.  They had taken her to her doctor who told them that she needed the additional supervision a nursing home could provide.  The family wondered what else could be done…

Then they heard that the Care Managers at Q4 were experts at evaluating the client’s health, functional abilities, home safety, and desires.  After completing thorough assessments our Q4 Care Manager discovered the root of the problem.  Our client’s confusion was related to her lack of oxygen and her falls related to the difficult surfaces and lack of safety equipment in her home.  The Q4 Care Manager worked with the physician to obtain the appropriate testing and then ensured the appropriate medications and oxygen were in place.  Working with a physical therapist, we improved her balance and core muscles.  Simple modifications were made to the home to improve safety and reduce the number of falls.   These simple changes made over a short period of time brought about significant results.  The things that were putting her health in jeopardy were not identifiable until her home, her functional abilities, and her health were evaluated simultaneously. 

Today she is enjoying her life.  She began playing bridge and going out for dinner again.  There have been no more falls, she has increased energy, and her mental status remains clear.  Her Q4 Care Manager visits weekly to ensure that subtle changes are identified and that interventions are begun immediately.  Her family has peace of mind and has security in knowing that things are being taken care of.  Safety, health, and home…let Q4 improve your quality of life.
